Incident Report: Ensuring a Safe NAK Brotherhood

Nu Alpha Kappa Fraternity, Inc. (NAK) is dedicated to fostering a safe, respectful, and inclusive brotherhood across our chapters in California, Colorado, and Nevada. Our Conduct Policies, including the Anti-Hazing Policy, Sexual Misconduct Policy, and Risk Management Policy, reflect our zero-tolerance stance on behaviors that compromise our values of academics, culture, and brotherhood. Use the form below to report concerns about a NAK chapter or member, such as policy violations, safety issues, or misconduct. Your report helps us maintain a safe environment and uphold our mission.

Reporting a Concern

Use this Incident Report form to document concerns related to a NAK chapter or member. Reports can be submitted anonymously by omitting contact information, but providing details enhances our Risk Management Review Committee’s ability to investigate thoroughly. Please answer each question with as much detail as possible, indicating “N/A” if information is unavailable.
